THE OPPORTUNITY:

Nurse Advisor(Office Based)-Kent

Their aim is to help patients improve their quality of life and the ability to self manage their conditions. They do this by working with patients in devising a well functioning health and social care pathway so that patients truly understand all aspects of their condition or disease.

THE RESPONSIBILITIES:

The overall responsibility within this role is to help high risk patients increase their understanding of their condition and to assist them in managing it better. This is a telephone/office based nursing role.


YOUR CLINICAL RESPONSIBILITIES:

Reporting into senor members of staff you will be responsible for ensuring the right information is provided to patients at all times. You are responsible for your own diary and caseload and will meet and work with each patient to jointly develop an action plan as to how their health and quality of life can be improved. You will coach them on how to implement their new care plan into their daily lives and to make pro-active choices. You will have an initiate ability to quickly identify any potential risks to patients and act accordingly. Part of your role will involve communicating with other health, social care and voluntary sectors ensuring that patients have the full support of the services. You will participate in weekly meetings with the team to plan, develop and implement changes to improve the service being offered to patients. You will be expected to keep up to date with current practices and solutions that will help patients improve their care of life, through a research based methodology, working within approved ethical guidelines. You will also be asked to assist in the training of new staff members and other ad hoc duties may be assigned to you by more senior members of staff.

YOUR EDUCATIONAL RESPONSIBILITIES:

Nursing Degree
NMC Registration


THE REQUIREMENTS:

To be eligible for the role, you will need to:

• have at least one years' experience working as a nurse with experience in assessing, planning and evaluating care and care pathways.
• Ideally have experience of working with patients with long term/chronic conditions
• have a good understanding of data protection, patient confidentiality and data governance, along with an understanding of risk management and health and safety issues.
• Be a caring and compassionate nurse with strong attention to detail and the ability to work well under pressure.
• have excellent communication skills and be a good team player.
• Have good experience working with Microsoft Office (Word, Excel, Powerpoint)
