On this page you will find information on how to request various of letters or forms you may need in relation to your employment with TTM Healthcare Solutions (TTM).
Important Notice
Please note that under General Data Protection Regulations (GDPR) TTM are very happy to facilitate your request and will do so as soon as possible. However, it is the policy of TTM to only accept letter requests and verification of service requests directly from current or past employees. TTM does not provide information directly to third party institutions or bodies requesting information. We adhere strictly to this policy. If you are a third party institution or body and require information, please send this link to your candidate and ask them to make this request directly.

Statement of Employment Letter
A Statement of Employment outlines the details of an employee’s employment with TTM. It explains the role that the candidate either has worked in or currently works in and how long they have been working for TTM (start & end date). Click here to request.
Reference Request
As TTM is a recruitment agency and does not directly supervise employee’s, we are not able to provide clinical references or comment on employee related matters. However, we are happy to provide a statement of employment to confirm your employment. Please request a Statement of Employment at the link above.
Work History Letter
A Work History Request outlines the details of an employee’s employment with TTM. It explains the role that the candidate either has worked in or currently works in, how long they have been working for TTM (start & end date) and also contains a record of all the hours worked by the candidate during their time with TTM. Verification of Services Form
A Verification of Services form outlines the details of an employee’s employment with TTM. It explains the role that the candidate either has worked in or currently works in, how long they have been working for TTM (start & end date), their point of scale and next increment date.
